Design flaw of early models[edit]

Can someone with more information about the design flaw of early models add some mention of it? I have seen some mentions of XM Radio having to launch replacement satellites early than originally planned. "generic failure of the early BSS-702 model: the fogging of the concentrator mirrors on the solar arrays leads to reduced available power."[1] How was the flaw corrected and how was it diagnosed?
