Čeština: Vlajka obce Moldava, okres Teplice, Ústecký kraj.
Modrý list s žerďovým červeným pruhem širokým jednu třetinu délky listu. V pruhu třikrát pravoúhle lomený bílý pruh vycházející z druhé šestiny horního okraje do druhé šestiny dolního okraje listu a dotýkající se svými vrcholy okrajů červeného pruhu.
Deutsch: Die Flagge der tschechischen Gemeinde Moldava im Bezirk Teplice, Region Ústecký, seit dem 5. April 2001.
